在规范化培训中培养中医临床思维的思考与探索*

摘要: 中医临床思维培养是中医住院医师规范化培训的重要内容,贯穿于中医规范化培训的整个过程。中医临床思维培养是一个较长时期内逐渐训练形成并强化、提高的过程,夯实理论基础是培养中医临床思维的前提,以整体观和辨证论治作为中医临床思维培养的核心,在培养过程中要注意与西医临床思维进行区分,要在中医四诊与病历书写、教学查房、病例讨论与医案学习、师承教育等环节中训练、强化和提升中医临床思维,并在临床实践中得以运用和检验。

关键词: 住院医师, 规范化培训, 临床思维

Abstract: The cultivation of clinical thinking is an important part in the standardized training for TCM resident doctors. It not only penetrates the whole training process but also forms a reinforced learning cycle over a long period of time. In this process, to consolidate theoretical foundation is the precondition and to develop a holistic view as well as syndrome-differentiation perspective are the core of cultivating clinical thinking. It is necessary to identify the differences between western and Chinese clinical thinking patterns. At the same time, four diagnostic methods, medical record writing, ward inspection, case discussion and learning, master-apprentice teaching mode should be particularly reinforced to the enhancement of clinical thinking.
